Private Mailbox Application

You will need to complete a Form 1583 from the United States Postal Service. Here is a template for Form 1583 to help you complete it accurately. This form MUST be notarized. If you would prefer completing this form in person with us, then you will not need to notarize the form. You must schedule an appointment with us to make this happen.

We will also need two types of identification. One must contain a photograph of the addressee(s). Acceptable identification includes: valid driver's license or state non-driver's identification card; armed forces, government, university, or recognized corporate identification card; passport, alien registration card or certificate of naturalization; current lease, mortgage or Deed of Trust; voter or vehicle registration card; or a home or vehicle insurance policy. A photocopy of your identification may be retained by our community manager for verification.
UNacceptable identification includes: social security cards, credit cards, and birth certificates.

Once we receive your application form then we will send you a reminder email to collect the documents needed to activate your mailbox membership. Once all documents and the membership fee have been collected, we will assign you a Private Mailbox (PMB) # so that you can start receiving mail immediately.

Private Mailbox Membership is $35 per month and due at the 1st of each month.
